The American College of Neuropsychopharmacology (ACNP) is a professional, international organization of leading brain scientists. ACNP is a non-profit, professional society dedicated to advancing the scientific understanding of and facilitating communication about disorders of the brain and behavior in order to advance their prevention and treatment. The membership of the College is drawn from scientists in diverse subfields of neuroscience, including behavioral pharmacology, clinical psychopharmacology, epidemiology, genetics, molecular biology, neurochemistry, neuroendocrinology, neuroimaging, neuroimmunology, neurophysiology, neurology, psychiatry, and psychology. The ACNP encourages participation in College activities by members of organizations whose mission and purpose is primarily to:

  • Advocate for improved treatment for people with brain, behavioral, and substance abuse disorders.
  • Advocate for the elimination of stigma associated with those disorders.
  • Provide assistance and support for people with brain, behavioral, and substance abuse disorders through referral services, self-help support groups, and other similar services.
  • Who share a common commitment with the ACNP for the support of scientific research in neuropsychopharmacology and related fields.
  • And who seek and use the advice of ACNP members on their scientific advisory boards.

The principal functions of the College are research, education, to offer investigators the opportunity for cross-disciplinary communication and to promote the application of various neuroscientific disciplines to the study of all forms of mental illness and their treatment. Their educational goals are to encourage young scientists to enter research careers in neuropsychopharmacology and to develop and provide accurate information about psychiatric disorders and treatment. The makeup of the ACNP allows us to offer expertise in the science of psychiatric disorders from the standpoint of diagnostics and therapeutics.

Rachel Waddington is an experienced independent international trainer and consultant specialising in innovative ways of supporting people who struggle with extreme states (including ‘psychosis’, ‘dissociation’ and post traumatic reactions). As well as having over 14 years of experience within the adult mental health field, I also have particular expertise in working with children, young people and people in prison who hear voices/see visions. Drawing on and sharing her own experiences, Rachel’s workshop on taboo and violent voices provides an insightful and illuminating introduction to topics that are usually considered off-limits. Her honest approach and the stimulating discussion she generated certainly expanded our group’s understanding and empathy for those dealing with such voices. In conjunction with Open Dialogue UK and an NHS trust, I am also undertaking Open Dialogue work as part of my 3 year training as an Open Dialogue Practitioner.

ECNP aims to:

  • Support innovative research in the convergent disciplines of neuropsychopharmacology and facilitate the communication of ideas, discoveries and best practices.
  • Encourage the scientific activities of countries in Europe and co-ordinate the development of common European standards.
  • Promote the entry of Early Career Scientists into the field and support their ongoing professional development.

To achieve its aims, ECNP organises a wide range of activities, programmes and events across Europe. These include:

  • Meetings: the annual ECNP Congress is attended by more than 5,000 psychiatrists, neuroscientists, neurologists and psychologists from around the world each year for the communication and cross-fertilisation of research results and ideas in the field of neuropsychopharmacology.

ECNP Networks bring together researchers with different expertise across Europe to foster successful collaboration and sharing ofideas, discoveries and practices in translational neuroscience. The overarching aim of ECNP Networks is to facilitate the collection of essential biological, clinical and therapeutic data in a standardised way to guarantee their robustness, reproducibility and translatability. KarXT is Karuna's lead product candidate that selectively activates muscarinic acetylcholine receptors in the brain to unlock the therapeutic potential of xanomeline, which previously demonstrated significant benefits in Phase 2 studies in schizophrenia and Alzheimer’s. Karuna assessed the potential of over 7,000 possible combinations of muscarinic receptor agonists and antagonists to find an optimized combination that could preferentially stimulate muscarinic receptors in the CNS to improve the symptoms of psychosis, while avoiding stimulation of muscarinic receptors in the peripheral tissues and the associated side effects. As a result of our research, we identified xanomeline and trospium as the most promising pairing for development in the form of KarXT (Karuna-xanomeline-trospium). Trospium is a potent and effective muscarinic receptor antagonist that does not measurably cross the blood-brain barrier, confining its effects to peripheral tissues, and it currently marketed for the treatment of overactive bladder in the U.S. and other territories worldwide. Karuna co-founder and chief operating officer, Andrew Miller, Ph.D., was responsible for identifying the initial hypothesis and driving the execution of the completed Phase I studies supporting the combination of xanomeline and trospium.

Despite xanomeline’s promising therapeutic benefit in treating psychosis and related behavioral symptoms in patients with schizophrenia and AD, its potential has been limited by cholinergic side effects, which are believed to result from the stimulation of muscarinic receptors in peripheral tissues. Karuna believes that the above data, as well as our Phase 1 and 2 clinical trials with KarXT demonstrating robust efficacy and significant reductions in the adverse events associated with xanomeline, support the further development of KarXT in multiple neuropsychiatric disorders, including schizophrenia and dementia-related psychosis.

Current antipsychotic treatments rely on the same primary mechanism of action (MOA) as they did when the first antipsychotic was discovered in the 1950s: inhibiting D2 dopamine receptors. Current antipsychotics are often used by physicians to address a wide range of neuropsychiatric disorders in addition to schizophrenia, including bipolar disorder and psychotic depression, as well as psychosis and agitation in elderly patients with dementia, but are associated with modest efficacy and significant side effects.

Muscarinic receptor agonists emerged in the 1990s as a promising innovative approach for treating psychosis and cognitive impairment. Muscarinic receptors are g-protein linked receptors (GPCRs) that bind the neurotransmitter acetylcholine. There are five distinct muscarinic receptors, M1-M5, found in the brain as well as various peripheral tissues.

The link between muscarinic receptor stimulation in the CNS, particularly stimulation of M1 and M4 receptors, and the reduction of psychotic symptoms and cognitive impairment, has been well studied and is supported by data from preclinical studies and randomized, double-blind, placebo-controlled clinical trials with xanomeline published in peer reviewed journals. However, the successful development of a therapeutic agent targeting muscarinic receptors has been limited by undesirable side effects that are believed to arise primarily as a result of stimulation of muscarinic receptors in peripheral tissues. We believe a therapeutic agent that can preferentially target and stimulate muscarinic receptors in the CNS, but not in peripheral tissues, has the potential to treat psychosis in schizophrenia and AD, including the associated agitation in patients with AD. We also believe the preferential stimulation of M1 and M4 muscarinic receptors in the CNS may address the negative symptoms of schizophrenia, such as apathy, reduced social drive and loss of motivation, as well as cognitive deficits in working memory and attention, all of which currently lack any approved treatments. This approach has the potential to produce a differentiated therapy relative to current D2 dopamine receptor-based antipsychotic drugs and to beneficially impact the lives of millions of patients with schizophrenia and other psychotic and cognitive disorders.

The EPIsoDE Study

The EPIsoDE Study is being conducted by the Central Institute of Mental Health (CIMH) in Mannheim, Germany (Principal Investigator: Prof. Dr. med. Gerhard Gründer). A second study center has been established at Charité Universitätsmedizin Berlin. The study is funded by the German Federal Ministry of Education and Research (BMBF). The MIND Foundation is an official study partner providing expertise, personnel, and additional funding.

The acronym “EPIsoDE” (Efficacy and safety of Psilocybin In treatment-reSistant majOr DEpression) summarizes the aim of the study: To investigate whether psilocybin administered under supportive conditions in a controlled, randomized, double-blind design is a safe and effective treatment for depression patients whose symptoms cannot be adequately reduced with antidepressants. Over the course of the study, 144 patients will be treated in Mannheim and Berlin. The rationale and study design are explained in detail in a recent article published in the scientific journal Neuroscience Applied.

Seroxat is a second generation SSRI (Selective Serotonin Re-Uptake Inhibitor). SSRI’s work by inhibiting serotonin re-uptake transporters thereby increasing serotonin levels in the brain. Serotonin is a chemical which affects the mood. By boosting that chemical the patient should feel better.
