The Alternative Future Group (AFG) is a charity that works locally in the community and through their Treatment and Recovery Centres to provide a wide range of innovative care services. They work with learning disabilities, physical disabilities, mental health concerns, substance misuse issues, complex care, autism, dementia and young people in transition to deliver life-enhancing person centred support to people with a diverse range of care needs
AFG's supported employment projects aims people with learning disabilities and mental health needs to achieve and maintain meaningful employment as everyone else.They work in partnership with Mencap and Pure Innovations to help individuals look for meaningful employment opportunities including paid, voluntary, work experience placements and traineeships. They help people develop employability skills and we build the confidence and social skills they need for work life through
- Job profiling – working with individuals to find out what they want to do and identify their abilities, skills and talents.
- Job seeking – working with individuals and employers to find the right job
- Filling out application forms
- Creation of CV’s and covering letters
- Interview preparation
- Travel training
- Supporting individuals on the job
- Job coaching
- Advice on benefits
- Access to training and educational opportunities
AFGs Supported Living provide living solutions to people with different levels of housing support and care needs including young people in transition, people with autism, learning disabilities, physical disabilities, substance misuse issues, mental health and complex care needs. Supported Living aims to equip people with the essential skills needed to stay living independently in their home of choice for as long as possible and lead a full and active life by helping someone remain in their existing tenancy, find a new home or make the transition from residential care or their family home into independent living. Support is provided in a variety of settings including individual homes or flats, shared housing/accommodation, extra care schemes and individual outreach/floating support services. There are numerous ways AFG are able to support individuals to keep well, safe and lead a fulfilling life as part of their community such as:
- Support to enhance an individual’s daily living skills including cooking, cleaning and shopping.
- Provide support to maintain tenancies including budgeting, setting up utility accounts, furnishing new properties, gaining home insurance, carrying out regular health and safety checks, understanding and following the terms and conditions of tenancy agreements and holding regular tenant meetings.
- Apply for and obtain appropriate benefits
- Support to help people keep their home safe and secure
- Support people to be more inclusive within their community by helping them to make friends and develop relationships, access local/wider community facilities, leisure activities, training and employment opportunities.
- Support independence by enabling them to make their own choices and decisions about life.
- Develop the person’s independent travel skills where appropriate
Alternative Futures Group supports young people who are moving from children’s services such as residential care homes and school into more independent living. Their models are based on the Pathways to Getting a Life and focus on work, housing, good health, friends and relatives and community participation.to support people in the transition from children’s services into independent living within the wider community.
Alternative Futures Group’s support planning process looks at a person’s whole life and how best to support them to achieve their aspirations, goals and outcomes. They work closely with the person supported, those who care and know them best, and other professionals using a variety of Person Centred Tools. This process creates their Person Centred Plan, identifying clearly how the person would like to be supported, what their needs are and what good support looks like for them now and in the future. In preparing young people for adulthood AFG helps in different ways including:
- Support to live independently
- Finding a job
- Money and budget management
- Getting involved in the community and developing personal relationships
Treatment & Recovery Centres provide support that draws on a wide range of expertise including Registered Nurses, Clinical Managers, Physical Health Practitioners, Advanced Nurse Practitioners, Forensic Specialists, Pharmacists, Physical therapists, Horticulture therapists, Art therapists, Psychologists, Psychiatrists , Peer and Recovery/Support Workers qualified from certificate to Masters doctorate level . AFG supports people to:
- Obtain the right treatment to stabilise their condition and keep them safe
- Develop new skills, knowledge and interests so they can get their lives back on track
- Facilitate their support plan with Community Specialists so they can stay well in their local communities
- Facilitate access to social, employment and education services
